Chart Review (Chart Review Brochure)

PROGRAM SUMMARY

The Chart Review program offers medical providers who receive Ryan White CARE Act funds an opportunity to determine how well standards of care are being met in their clinical practices. Using a team of specially-trained AETC faculty, a review of randomly selected patient charts is completed to identify the strengths of the health care team as well as areas of opportunity for quality improvement.
The program is designed to be a useful learning experience, with no punitive repercussions.

Training, consultation, and educational materials are offered to support continued excellence in care.

All participating faculty have completed a confidentiality agreement and HIPPA standards are always met. There is no charge to Ryan White CARE Act clinics for Chart Review.

ACCOMPLISHMENTS

The Florida/Caribbean AETC has conducted Chart Review programs in various locations throughout Florida, Puerto Rico, and the U.S. Virgin Islands since July 2004.

Chart Review
Depending upon the size of the clinic, the team performs Chart Reviews in one or two days.

Case Conference
Following the review, a clinical case
conference is conducted. Clinical providers present a case for discussion with the AETC faculty. CMEs for physicians and CECs for nurses are provided at no cost.

Feedback Session
The strengths of the health care team are identified. Areas that could be strengthened resulting in improved health care for the patients are also discussed. Pocket treatment guides, drug cards, sample chart tools, HIV CareLink newsletters, and chart forms are made available.

Reporting
Results of the Chart Review process are compiled and shared with the clinical staff and administrators, usually within one week. Summaries and suggestions discussed in the feedback session are also included in the report.

Continued Support
Training opportunities or consultation sessions can be scheduled during, or at any time following the Chart Review. A follow-up review is conducted after one to two years to assess improvement.
